We are all in a shelter in place or soon to be a full quarantine.  It can be a time where we can overcome differences, be more understanding and find a deep caring for each other more than ever.  This is a time where relationships will be tested.  It is a time to be realistic.  If your relationship is clearly failing, it’s time to move.  Courts are now closed to the public and you will need to use an attorney to make sure you can file and that you can get the process started, efficiently and effectively.

 

 

Divorce Questionnaire

  • Please state your name and date of birth.
  • Please identify where you are currently living.
  • Please state the name and dates of birth for each child born of the marriage
  • Please identify the date of your marriage and where you were married
  • Please state your current job and gross annual income
  • Please provide the name of your spouse and his/her date of birth
  • Please provide your spouse's current job and his/her gross yearly income
  • If you were separated, what is the date of separation?
  • Has either Party filed a court action? If so, please state what was filed and where. If there is a current pending hearing, please state the day and time.
  • Please state all of your current credit cards, personal loans or other debt, etc.
  • Please list current fair market values of all assets
